{"id":1501,"date":"2009-10-15T00:25:18","date_gmt":"2009-10-15T07:25:18","guid":{"rendered":"http:\/\/christianfighterpilot.com\/blog\/?p=1501"},"modified":"2009-11-21T21:17:37","modified_gmt":"2009-11-22T05:17:37","slug":"report-maintenance-caused-fatal-t-38-crash","status":"publish","type":"post","link":"https:\/\/christianfighterpilot.com\/blog\/2009\/10\/15\/report-maintenance-caused-fatal-t-38-crash\/","title":{"rendered":"Report: Maintenance Caused Fatal T-38 Crash"},"content":{"rendered":"<p>The <a href=\"http:\/\/www.af.mil\/news\/story.asp?id=123172499\">Air Force has released the report<\/a> of the Accident Investigation Board (AIB) for the <a href=\"http:\/\/christianfighterpilot.com\/blog\/2009\/05\/21\/t-38-crashes-at-edwards-afb\/\">May 2009 fatal crash of a T-38<\/a> at Edwards Air Force Base.\u00a0 The crash killed the front seat pilot, Capt Mark P. &#8220;Dash&#8221; Graziano, and severely injured the rear seat engineer, Maj Lee V. Jones.\u00a0 Both were students at the USAF Test Pilot School.<\/p>\n<p>The <a href=\"http:\/\/www.christianfighterpilot.com\/articles\/files\/t38aib.doc\">publicly-releasable AIB report<\/a> indicates that the T-38 crashed because <!--more-->the rudder failed in a hardover condition.\u00a0 The uncommanded, full-deflection rudder caused the T-38 to go violently out of control.\u00a0 The board found &#8220;substantial evidence&#8221; that the\u00a0unrecoverable failure\u00a0was likely the result of a\u00a0maintenance error.<\/p>\n<p>The report was highly critical of the maintenance oversight, as noted in the article:<\/p>\n<blockquote><p>The report stated that &#8220;insufficient supervisory oversight and a lack of discipline of the training process&#8221; was a factor.
